Our Origin Story

From Vancouver General to a Care-Coordination Platform

TEAM of Care is the pioneer of active care management. The company launched in 2012, but the foundational algorithm began in Vancouver in 2009, when we built a care coordination and treatment lifecycle management protocol for Vancouver General Hospital. By incorporating best practices defined by VGH cardiologists, the model became the Heart Failure strategy for all of British Columbia.

We modified and automated those care coordination rules with process automation and a rules engine, and they now serve as the foundation for TEAM-Win. Today, our platform combines best-practice operational processes with the most advanced healthcare technology available to deliver real-time, evidence-based care coordination across a diverse team of providers.

Mark Stabile, JD, MPH

Chief Executive Officer

Mark has a passion for driving systematic change in healthcare. Before TEAM of Care, he founded Value Catalyst, where he built the care management model that became the foundation for our software. Before that, he led Greencastle Consulting for 12 years, implementing large-scale transformational health IT programs. Mark began his career as a U.S. Army officer and Airborne Ranger. B.S. in Economics from West Point; J.D. from Seton Hall Law; MPH in Health Services Management from Columbia.

Thomas McCarter, MD, FACP, FCPP

Chief Medical Officer

Board-certified internist and the country’s leading physician expert witness on medical necessity. Dr. McCarter joined TEAM of Care from Executive Health Resources (an Optum company), where he developed the clinical rules used to adjudicate over 10 million transactions across 2,500+ U.S. hospitals. He previously served as CMO of Main Line Health System (5 hospitals) and VP of Medical Affairs for Tenet’s Pennsylvania region (8 hospitals). MD from Hahnemann; biology and chemistry degrees from UNC Chapel Hill as a Morehead Scholar.

Jim Sheils

Executive Vice President for Growth

Healthcare evangelist and business developer with a track record across startups and established companies. Jim thrives in agile environments — defining new markets, building strategic partnerships, and turning ideas into scalable solutions. He brings a laser focus on customer and partner satisfaction and a strong grasp of how people and organizations actually behave. Engineering and project management foundation from West Point and Villanova.

Mike DeCaro, MS

Principal Software Engineer

Mike leads the full-stack development of TEAM-Win. A hands-on engineer since 2008, he combines deep expertise in modern platforms, distributed systems, and large-scale application design with strategic thinking honed across the finance and healthcare sectors. Bachelor’s degrees in Computer Science and Applied Mathematics; M.S. in Computer Science from New Jersey Institute of Technology.
