team of care
About
Accelerating the access to and application of knowledge throughout the continuum of care.
Our Origin Story
From Vancouver General to a Care-Coordination Platform
TEAM of Care is the pioneer of active care management. The company launched in 2012, but the foundational algorithm began in Vancouver in 2009, when we built a care coordination and treatment lifecycle management protocol for Vancouver General Hospital. By incorporating best practices defined by VGH cardiologists, the model became the Heart Failure strategy for all of British Columbia.
We modified and automated those care coordination rules with process automation and a rules engine, and they now serve as the foundation for TEAM-Win. Today, our platform combines best-practice operational processes with the most advanced healthcare technology available to deliver real-time, evidence-based care coordination across a diverse team of providers.

Why Us
Healthcare Is a TEAM Effort
High-performing healthcare organizations do cross-functional and cross-enterprise work to serve their patients through value-based care, payer-provider joint ventures, or refined care coordination across Healthcare Systems. Is there a single playbook for everyone involved? Does every team member know their part, what’s expected, and the status at any given time? We make that possible.
What Makes Us Different
Cross-System Orchestration
We connect the standalone clinical, financial, and administrative system workflows you already run into a single process — woven across those systems but with visibility across the entire enterprise. No rip-and-replace.
Cross-Organizational Orchestration
We connect departments and organizations, creating the processes that don’t yet exist — to remove the barriers to success at the boundaries and handoffs between stakeholders.
Industry-Leading Results
Over $200M in customer savings and VBC revenue. Performance isn’t luck — every process is configured to support specific, tangible goals and metrics.
Rapid Design and Deployment
Our open-source BPMN process modeler plus proprietary configuration tools connect to EHR and other system data. Fully operational enterprise processes in hours and days, not months.
Closed-Loop TEAM-Work and TEAM-Management
Nothing falls through the cracks. Everyone has visibility to tasks and status. When priority changes, the system automatically escalates based on your business rules.
Our Philosophy
Three Essential Questions
The 5 Rights are the tools. The philosophy below is how we use them. Better outcomes and lower costs require active management of care — and TEAM-Win helps you answer three questions.
What work must be done to achieve your goals?
The TEAM-Playbook defines tactics as process flows with performance metrics, then automates them into user actions.
Is the planned work getting done?
TEAM-Work codifies and automates the work to be done — giving real-time visibility to team members and managers, with the ability to swiftly reallocate work when bottlenecks appear.
Is that work making an impact?
TEAM-Management ties process measures directly to outcome measures. If something isn’t working, update the Playbook and redeploy in days.
The Execution Discipline
The Five Rights of Execution
High-Performance TEAMs Know How to Execute their Plans to Reach their Goals.
01
Right Plans
Start with goals and align them to the realities of your organization.
02
Right TEAM
In value-based care, the right team spans multiple organizations and clinical, operational, and financial disciplines.
03
Right Work
Focus on activities that reduce cost and improve quality — not work that just fills time.
04
Right Resources
Make the data, tools, and authority needed to act available at the point of work.
05
Right Results
Tie process measures directly to outcomes, so you know whether the work is making the impact you intended. This is the discipline behind every TEAM-Playbook we build.
High-Performing TEAMS
What High-Performing TEAMs Share
High-performing healthcare organizations do cross-functional, cross-enterprise work to serve their patients. We make that possible — by design.
01
Predictable Actions
Plan the work and work the plan. Everyone operates from one Playbook — by design, every team member knows the right work to do and how to do it well.
02
TEAMWORK
Everyone does their part. Members know their role, get work done, communicate well, and support each other at the handoffs.
03
Strong Management
Know that the right work is being done, the right way, at the right time — and is making the right impact.
