    Healthcare has entered a new era of integrated care delivery and accountability. Physicians are now expected to manage patients when they are healthy, when they seek specialized treatments, and as they move between hospitals, nursing homes, and home care services. To realize the promise of integrating the entire care continuum to enable population health management and successful participation in one or multiple Fee For Value Initiatives requires active care coordination and effective communication between all stakeholders. Active care coordination is not easy to achieve because there have never before been an incentive to connect the entire care continuum. As health systems grow their value-based contact portfolios, they must build their attributed population health capabilities. In this session you’ll hear how integrated delivery systems are building programs that improve active care coordination, apply analytical algorithms to improve risk stratification, feed actionable clinical, financial, and administrative data into clinical workflows and engaging patients through multiple channels. Examples of these initiatives include:

    • Shift to population health and reorganizing/unlearning the past financial performance incentives
    • Better understand and parse value-based care – What is it and how and why is it superior to volume-based FFS medicine?
    • Payor and financial trends in healthcare that will impact my department in the next year
    • Bridging the care gap and providing real time collaboration that connects providers and patients

    Accountable Care Organizations (ACOs) offer enormous opportunity for patients and providers to work together to increase quality of care, lower costs and improve the health of the community. The Patient Protection and Accountable Care Act of 2010 (ACA) opened up new opportunities for Medicare and Medicaid beneficiaries to take advantage of care in an Accountable Care Organization and gave ACOs new opportunities to serve this important population. ACO’s from around the country have come together to form NAACOS with the following mission:
    Foster growth of ACO models of care;
    Participate with Federal Agencies in development & implementation of public policy;
    Provide industry-wide uniformity on quality and performance measures;
    Educate members in clinical and operational best practices;
    Collectively engage the vendor community, and
    Educate the public about the value of accountable care.

    The Strategy, Structure, and Systems You Need to Get Value Based Care Right

    The shift to value-based reimbursement is happening and to date over 300 Medicare ACOs have invested millions in population health and complex case management. Yet this year we saw ten of the original 32 Pioneer ACOs drop out of the program, showing that the shift is not easy even if your organization appears ready. As providers determine how they will participate, either as an ACO or otherwise, the IT department will be a key player asked to deliver the information and analysis needed to support the collaboration and reporting required. In this session, Health System IT leaders who were early innovators, some who have achieved significant shared savings through the ACO program reveal lessons learned and discuss how to:

    • Streamline data capture from physician practices at different levels of tech adoption
    • Set up an organization-wide structure to share data and use that information to deliver quality care
    • Build and implement tools to report on ACO metrics, identify gaps in care, and reach out to high-cost patients on a regular basis
    • Set up the optimal ACO with private and public HIEs playing a role, capturing data from various sources and using that patient data to deliver high-quality, cost-effective care
